If you are unable to go out unaccompanied and have no friends or family living nearby, we can request patient transport for any hospital appointments.
Please give us as much notice as possible to arrange this for you – at least 48 hours (2 working days) is required or a transport request will not be accepted.
To arrange transport we will need the following information:
- The date, time and location of your appointment (including hospital department)
- What assistance you require – do you use a walking aid or travel in a wheelchair?
- Main diagnoses/problems – in particular, those affecting your mobility
- What access restrictions there are at your address – ie. number of stairs
- Whether you require an escort to accompany you (friends/family members may have to make their own way to the appointment unless there is a medical reason they should go with you)
Please submit a Patchs request including as much information as possible, or call the Reception team after 10am to make a request for patient transport.
